The Caregiver's Companion allows caregivers to organize and access pertinent medical information regarding their loved one or patient. It allows the caregiver to focus more on the individual and less on remembering dates, diagnoses, medications and reactions. Now, the caregiver can de-stress, confident in the fact that this information can be found in one convenient place. In the absence of a caregiver, this medical history organizer also helps the forgetful patient who needs a little assistance in remembering the date of that last flu shot, the doctor's name and phone number or the generic name of that new medication. Now everyone can rest a little easier at the doctor's office knowing that this information is only a page turn away.

JoAnn Funderburk, librarian and author, is married with two adult children and two grandchildren. Several years ago she found herself sandwiched between taking care of her own children while also seeing to the care of an aging parent. The Caregiver's Companion pamphlet culminated from being suddenly thrust into the caregiver position for her mother after she had a massive heart attack and could no longer care for herself. After just a few doctor's appointments, JoAnn saw the need for collecting her mother's medical history in a place where information could be easily stored and updated as needed. Originally kept in a small spiral notebook, medical staff strongly encouraged JoAnn to publish this information in a format where more people could benefit. The Caregiver's Companion: a Medical History Organizer is the product of their encouragement and her creativity.
