Vectorcardiography is a method of recording, in the form of a continuous loop, the magnitude and direction of the electrical forces produced by the heart. The vectorcardiogram, or VCG, retains the phase relationships between leads and hence allows certain electrocardiographic abnormalities to be more clearly displayed than does the conventional 12-lead ECG. The technique also helps to discriminate between various diagnoses when the conventional 12-lead ECG interpretation may be uncertain.
The traditional method of recording the vectorcardiogram is to use a completely different set of electrodes compared to recording the conventional 12-lead ECG. However, the recent availability of equations which allow the vectorcardiographic XYZ leads to be derived from the conventional 12-lead ECG has introduced a new era where both the 12-lead ECG and the vectorcardiogram can be derived from the same lead system. The authors have termed the VCG, derived from the conventional 12-lead system, the '12-lead vectorcardiogram'. Although in the past the recording of a VCG has been a time-consuming process, current technology enables a VCG to be derived and displayed in seconds on multi-channel ECG machines such as are in daily use throughout the world.
12-lead Vectorcardiography is a unique text on the theory and practice of this branch of electrocardiography. It is divided into seven sections. Initially, stress is laid on lead systems and, in particular, the derivation of the XYZ leads and the vectorcardiogram from the 12-lead ECG. Clinical sections dealing with normal appearances, ventricular hypertophy and myocardial infarction, etc. are presented. Many examples are contributed from the authors' own ECG databases and these are extensively reinforced by line illustrations. The authors place emphasis on the additional information which can be obtained from the VCG compared to using the 12-lead ECG alone.
12-lead Vectorcardiography will be of interest not only to cardiologists, physicians, nurses, ECG technicians and students of clinical physiology requiring an introduction to the topic but also to those who already interpret ECGs on a daily basis.
