About the Author:

Harold Merskey, DM, FRCP, FRCP(C), FRCPsych, did service in the Royal Army Medical Corps, and among his appointments worked as a psychiatrist at the National Hospital for Nervous Diseases, London (UK), and in Canada at the London Psychiatric Hospital and the University of Western Ontario.

John D. Loeser, MD, is Professor of Neurological Surgery and Anesthesiology at the University of Washington in Seattle. He is on the Attending Staff of the University of Washington Medical Center, Harborview Medical Center, and Children's Hospital and Regional Medical Center.

Ronald Dubner, DDS, PhD, is Professor and Chair of the Department of biomedical Sciences, University of Maryland Dental School.
