Isaac Adair has just begun his freshman year in college when strange psychological symptoms lead him to see psychiatrist Dr. Alison Wright. The doctor quickly diagnoses him with schizophrenia, and attempts to use medication and therapy to get him back on track. Isaac, in a state of paranoia murders not only the doctor but two of her co-workers. After a judge rules Isaac unfit to stand trial and places him in an institution for the criminally insane he makes it his one and only mission to escape. Months later on the run from the authorities Isaac is unable to remember what he has done and trusts no one. He believes Alison is still alive, and he must find her. With a dysfunctional childhood haunting him and the only force motivating him forward being false memories of a love affair with Alison, Isaac goes on a spree of heinous crimes. Fighting the demons from his past, and also the demons he now sees in hallucinations growing more vivid with each day. Isaac has to choose whether he wants to find a place in reality or stay in the complicated world his schizophrenia has made for him.
