Over the past 10 years, there has been a sea change in emphasis in the field of kidney disease, from a focus on management of kidney failure with dialysis or transplantation to identification of early kidney disease with the goal of increasing survivability by mitigating cardiovascular risk as well as slowing the rate of chronic kidney disease (CKD) progression. So the time was propitious to put together this Handbook of Chronic Kidney Disease Management that emphasizes care of nondialysis CKD patients in stages 1-5.
It was an enormous privilege and an intellectual feast to interact with the eminent contributors to this Handbook who somehow managed to find time in their busy schedules to distill for us their knowledge and expertise regarding all aspects of diagnosing, treating, and preventing progression of CKD.
It was our goal to develop a book that would be useful across the world, and special effort was made to recruit authors from various parts of the globe to better reflect the range of practices and problems encountered in different countries. Strict attention was paid to referencing not only U.S. guidelines, but also those in the United Kingdom, Europe, Canada, and Australia, and to express laboratory values using both metric and SI units.
In the course of 44 chapters, I believe that the goals of describing most aspects of CKD management have been largely achieved. This Handbook is intended not only for nephrologists, but also for the broad range of health care practitioners--internists, generalists including nurse practitioners and physician assistants, cardiologists, and endocrinologists--who care for early-stage CKD patients.
