All of us have information that only we know, but what will happen to that information - and everything you've worked for - if you become seriously ill or die? If you are unable to handle your affairs and your real and personal property are not properly documented, assets may be lost.

This workbook is designed to help you organize and catalog vital personal information, and it provides a place for you to record this information in case someone needs to act on your behalf. In emergency situations, many of us don't think clearly. Crises have a way of making our minds go blank. Take time to complete the pages in this workbook while your life is relatively calm. If you, or someone acting for you, then have to act quickly, essential information will be easily accessible. As you complete these pages, you may even find that you have forgotten an account or investment you own.

This 144 page, 8 1/2" x 11" softcover book, has a spiral binding to allow it to lie flat for easy writing, and is printed on bright white paper. It is designed to be easy to carry from place-to-place as you complete various inventories (to a bank safe deposit box, e.g.).

From the Author:

This book was initially written for my own use after the death of my husband. I wanted to ensure proper medical information would be relayed in an emergency, and that necessary bills would be paid, and proper people notified. Friends saw the book and strongly urged me to make it available to others.

About the Author:

Born and raised in Kingston, NY, I moved to the Houston area with a major computer corporation to work on the space program. Since taking an early retirement from that company, and since my husband's death, I've worked as a volunteer with Hospice, and am a member of their board of trustees. My dog, Lucy, and I volunteer with a pet therapy group at a local hospital. Additionally, I stitch models for a noted needlework designer, and love to travel.
