As Alzheimer s disease becomes a more prevalent problem amongst our aging population, this information will be invaluable for health care practitioners. Additionally, the book provides examples of goals and objectives for clients with specific problems. The problem areas are organized around the cognitive domain, the physical domain and the psycho-social domain. Within each one of these sections, examples of problems, goals and objectives, are provided. Specific activities are analyzed in order to illustrate how to utilize the measurements towards progress. This method of measurement of progress is what makes this book unique. The tools provided can potentially help providers develop programming which will address the needs of their clients. The tools and charts book will help the practitioner assess individual clients, observe the clients strengths and plot a course towards independence.Includes R.O.O.F forms with AMA Current Therapeutic Procedural Codes built right into therapeutic/ activity forms.
