Organized according to types of complications, the text explores pin tract infections, nerve and vessel injury, interference with limb function, failure to obtain union, unsuccessful arthrodesis, persistent wound sepsis, fixator problems, and post-fixation complications. The book features the author's unique zone system for pin and wire placement, which helps prevent serious neurovascular injury. Safe and unsafe areas for pin insertion are demonstrated through illustrations of cross-sectional limb anatomy.
About the Author: Dr. Green is an orthopaedic surgeon, whose subspecialty interest deals with post-trauma limb reconstruction, including non-unions, malunions, shortening and post-trauma infections of bones and joints. His 1981 book, “Complications of External Skeletal Fixation: Causes, Prevention and Treatment,” created a revolution in orthopaedic care throughout the world. Prior to the book’s publication, external fixation had a justifiably bad reputation among surgeons, a consequence of the unacceptably high incidence of pin tract infections, nerve and vessel injuries and non-unions of bone. Hence, with the exception of pin-in-plaster stabilization of certain fractures, mechanical external fixators were rarely used. Green’s book, by offering a logical and reliable method of avoiding the most worrisome fixator associated complications, brought external fixation into common usage by orthopaedic surgeons and traumatologists. As a result, fixators are today routinely used as the first method of stabilization of serious fractures of long bones. With the introduction of the methods of bone reconstruction and lengthening developed by Russian surgeon G. A. Ilizarov, the use of external fixation expanded beyond applications limited to post-trauma stabilization and repair. During the past 25 years, Dr. Green has lectured and published extensively on matters dealing with his area of expertise. He was the first American to visit Dr. G.A. Ilizarov in Kurgan, Russia and was instrumental in introducing the Ilizarov method of reconstructive surgery to the world's orthopaedic surgeons. He is the author four books, and the co-author of 15 others. He has more than 100 publications in peer-reviewed medical and surgical journals. Dr. Green is a founding member and past president of the Limb Lengthening and Reconstruction Society of North America. He has served as president of the Association of Bone and Joint Surgery. Green has been invited to lecture about these and related topics throughout the world. The year 2008 was the 75th anniversary of the American Academy of Orthopaedic Surgeons. To celebrate the event, the AAOS published a book on the history of American orthopaedic surgery called “Moving Stories” and produced a companion DVD titled “Moving Pictures.” Dr. Green’s role in our nation’s orthopaedic history is featured in both the book and the movie. Dr. Green has been actively engaged in research at two of UC Irvine's biomedical research labs. At the Biomechanics Lab at the Long Beach Veteran's Hospital, Dr. Green works with Thay Q. Lee, Ph.D. on problems related to the biomechanics of external skeletal fixation and internal fixation devices. In conjunction with Vincent Caiozzo Ph.D., Dr. Green studied the effect limb lengthening has on muscle tissue with the goal of the reducing the complication associated with limb elongation. Stuart A. Green, M.D. received his undergraduate degree from Lafayatte College and his M.D. degree from New York Medical College. He completed his surgical internship and residency in Orthopaedic Surgery at the Hospital for Joint Diseases in New York. Dr. Green completed fellowship training in Infectious Diseases at the University of California, San Diego. He currently serves as a Clinical Professor in the Department of Orthopaedic Surgery at the University of California, Irvine. Dr. Green is the Deputy Editor for Clinical Orthopaedics and Related Research; he has served as the Associate Editor for the Journal of Orthopaedic Trauma, the Associate Editor for Advances in Orthopaedic Surgery, and the Associate Editor for Techniques in Orthopaedics.
