Pilcher devotes much attention to comparative anatomy, "Deals very fully with structure of the ossicles, various birds and mammals, and he gives one of the earliest accounts of the otoliths in fishes . The surgical principles applied by Pilcher and other surgeons to otology ensured progress on scientific lines." (Stevenson & Guthrie, p.62)" Annotated Bibliography of the Dennis Pappas Otolaryngology Collection (2014), p.36 Georg Pilcher (1801-1855) "A lecturer in anatomy and surgeon at the Surrey dispensary, in 1838 Pilcher published the first edition of A Treatise on the Structure, Economy and Diseases of the Ear, for which the Fothergillian gold medal was awarded. It gave a general view of the anatomy, physiology and diseases of the ear. He wrote this treatise because "aural surgery is either almost entirely neglected, or for the most part is left in the hands of the ignorant empiric." Compared to the European treatises of Itard, Saissy and Kramer, it offered nothing new, but it was more scientific than the other English publications of most of his contemporaries, by Curtis, Wright, Turnbull, Stevenson and Webster. The well-illustrated summary of the comparative and descriptive anatomy of the organ of hearing also gave an impression of independent study. Concerning the presence of muscle in the tympanic membrane described by Home, Pilcher stated that "the most careful examination does not afford evidence of such a structure in the human subject." He then discussed the various possible malformations of the ear. He remarked that "it is a curious circumstance that deaf-dumbness is much more frequent in males than in females." He discussed at length the different forms of otitis, and foreign bodies in the external auditory canal. The "nervous diseases of the ear" were only superficially presented, and treated empirically in particular with "aetherous vapour into the tympanum" by catheterization of the Eustachian tube, "blisters behind the auricle", or local "citrine ointment".
