Un sistema sanitario verdaderamente controlado y gestionado públicamente con la participación comunitaria directa de las clases populares, que sirva al interés del bien público y donde nadie pueda hacer negocio con nuestra salud. Las desigualdades en salud han sido durante dos décadas un asunto ampliamente estudiado en las revistas científicas de epidemiología y salud pública, y de esos estudios se derivan conclusiones verdaderamente impactantes. Una de ellas es la estrecha ligazón existente entre salud y nivel económico, obvia si consideramos la salud de los que viven en la pobreza extrema y la de los que poseen rentas económicas elevadas, pero menos obvia cuando los pobres son menos pobres y los ricos menos ricos. Otro dato sorprendente es la constatación de que en una misma ciudad las expectativas de vida de sus habitantes y en general su salud pueden ser muy diferentes según el barrio en que viven, lo cual tiene lógicamente que ver con su estatus económico. Para quien no ha profundizado en el tema, puede resultar chocante constatar, por ejemplo, que la expectativa de vida y la salud en general de los habitantes del estado indio de Kerala son mejores que la de los habitantes afroamericanos de Chicago. A pesar de la abundancia de estudios sobre las desigualdades en salud, éstas aún permanecen como un tema en gran parte desconocido para muchos profesionales de la salud y las ciencias sociales, así como para el público en general. Tal y como se revisa extensamente a lo largo del libro, una de las razones más importantes de este desconocimiento es nuestra incapacidad para "ver adecuadamente". Esa ceguera selectiva, que escapa a nuestra conciencia, dificulta en gran medida entender las raíces de las desigualdades sociales y como éstas dañan a la salud colectiva. Debemos pues aprender a mirar. Los autores de este libro han sentido la necesidad de explicar la importancia de este tema a un público no especializado. Su objetivo es ofrecer a cualquier ciudadano los anteojos necesarios para ver, para entender uno de los retos más importantes que la humanidad encara en los inicios de un nuevo milenio.
