It is now quite out of the question to considcr a laboratory dealing with the analysis of biological materials that is not equipped with an extensive range of instrumentation. Recent years have also seen a dramatic improvement in the ease with which such instruments can be used, and the quality and quantity of the analytical data that they can produce. This is due in no sm all part to the ubiquitous use of microprocessors and computers for instrumental control. However, under these circumstances there is areal danger of the analyst adopting a 'black box' mentality and not treating the analytical data produced in accordance with the limitations that may be inherent in the method used. Such a problem can only be overcome if the operator is fully aware of both the theoretical and instrumental constraints relevant to the technique in question. As the complexity and sheer volume of material in undergraduate courses increases, there is a tendency to reduce the amount of fundamental material that is taught prior to embarking on the more applied aspects. Yet until recently, old people were systematically exduded from participating in drug trials and similar investigations. As a result, knowledge regarding incidence, natural history and management of depression in the elderly are somewhat limited. These are considered to be due to age-related physiological changes. In the first section, epidemiology, presentations and assessment of depression in old age and neuroendocrine abnormalities observed in patients with depression are briefly discussed. However, it mainly deals with the problems of antidepressant drug therapy, such as which antidepressant to prescribe, how to monitor efficacy and side effects, what is the adequate dose, how long to treat a single episode of depression and who requires long term medication. They formed a professional interest group called the Specific Interest Group in Bilingualism because of the need felt by speech therapists to have some forum for discuss ing the challenges posed by the assessment and treatment of the bilingually language handicapped. The single most signifi cant factor in holding up the creation of such a book was the lack of expertise in one person of all the areas of paediatric nursing. The problem was resolved by producing an edited book, covering various specialities of paediatric nursing. Undoubtedly, one could have included more detailed coverage of particular speciality areas, e.g. chapters on ethical issues surrounding adolescent care, child abuse, teenage mothers, behaviour modification programmes and so on. The approach we took, however, stressed the overall unity of paediatric nursing - where the specialities are wide enough to take in all paediatric nursing activities from neonatal care to adolescent health screening. I am certainly very aware that the book would not have been written and produced without the support, enthusiasm and co operation of the chapter-writers. Two important sub-types have been identified: Alzheimer's disease and multiple-infarct dementia. Alzheimer's disease or Senile Dementia of the Alzheimer Type (SDA T), arises out of changes in the brain which are as yet poorly understood and identifiable with certainty only at post mortem examination. This type of dementia has been named after Alois Alzheimer, who first identified these changes, in the earliest years of this century. Alzheimer's is the commonest type of identified dementia. The second commonest type is multi-infarct dementia (MID), which follows a stroke or strokes affecting the relevant part of the brain. Like SDA T, it can be diagnosed with certainty only after death, although Jorm (1987, Ch. 8) reviews progress in using various types of tests which can suggest, if not confirm, its presence during the sufferer's lifetime.
