The author surveys predisposing and exciting causes, including hereditary scrophula, a delicate chest, temperamental vascularity, infections like smallpox, uterine disturbances, and environmental factors such as cold, damp air, and occupations that expose the lungs to irritants. He notes potential tubercular tubercles as disease markers and debates contagion. The proximate cause is described as ulceration of lung substance; diagnosis requires differentiating phthisis from tussis stomachica, catarrh, asthma, vomica, empyema, and hepatic abscess.
