An Inaugural Dissertation On Pulmonary Consumption by Alire Raffeneau-Delile. This early 19th-century medical treatise defines phthisis pulmonalis (pulmonary consumption) and traces its history, symptoms, and stages from incipient cough to hectic fever and emaciation. It distinguishes two forms—dry (phthisis sicca) and moist (humida)—and discusses how early neglect or mismanagement of the cough leads to progressive inflammation, purulent expectoration, night sweats, weight loss, and death. The author surveys predisposing and exciting causes, including hereditary scrophula, a delicate chest, temperamental vascularity, infections like smallpox, uterine disturbances, and environmental factors such as cold, damp air, and occupations that expose the lungs to irritants. He notes potential tubercular tubercles as disease markers and debates contagion. The proximate cause is described as ulceration of lung substance; diagnosis requires differentiating phthisis from tussis stomachica, catarrh, asthma, vomica, empyema, and hepatic abscess. Treatment emphasizes early intervention: cautious bloodletting in inflammation, emetics, blisters, tonics, and a restorative diet; exercise, fresh air, and climate change; and, in advanced stages, supportive nutrition and palliatives.
