"It is a superb work with outstanding photos and diagrams. "I have a few suggestions for future editions (since you asked). The sections on meeting the challenges of radial loops and subclavian difficulties are great, but we always emphasize to our Fellows not to loose ground that was hard won. In other words, once the wire reaches the central aorta, do not loose this access. I was not sure this message was strong enough. "A comment about using an exchange wire to hold central access: Exchange wires are long and often require two people to handle (double the chance for error). The "Jet Technique" that allows a standard 190 cm wire to be used is very fast and requires only one person. I believe the Harvard Cath Labs use this technique. Jetting is when the catheter is jetted off the wire (left coiled in the RCA cusp) by injection of saline through a 10 cc syringe on the end of the catheter, allowing one to let go of the wire and pull the catheter off of it, yet leaving it positioned in the central aorta. The next catheter is then passed over this using the resistance of the aortic valve to prevent forward wire movement while the catheter advances. "Finally, my favorite topic is right heart/venous catheterization from the arm. We are doing almost all of our right hearts via the forearm these days (unless there are no veins or there is severe arm edema). It also allows pacemaker placement, if needed, for heart block, etc. I think this addition to the transradial tool chest is valuable.
