From Chapter 1: Communication: This is the most important reason for writing in the chart. You are a professional with a specific area of expertise. For the patient to benefit, you must communicate to other team members the assessment, plan, actions, and patient response. The chart provides for all the team members, who cannot meet face to face every day, an ongoing record.
