Biofilm persistence, anatomical variations, and the presence of accessory canals often compromise debridement in this area.From a clinical perspective, precise determination of the working length and controlled instrumentation are essential to prevent procedural errors such as ledge formation, transportation, or over-enlargement of the apical foramen. Irrigation protocols and advanced delivery techniques are necessary to enhance microbial elimination where mechanical shaping is limited. Successful obturation of the apical third ensures a fluid-tight seal, thereby preventing reinfection and promoting periapical healing.
