A single page (front and back) in good condition (one edge a little worn and darkened, one 1cm tear, the application blanks are unmarked). The front consists of several questions to be answered, followed by a list of reasons to deny the application. The back consists of little road diagrams, for the applicant to specify where the sign will be located.
