Battle of Local Anesthetics

Pain management is essential in dentistry, especially in pediatric patients where fear and anxiety can strongly influence treatment behavior. Local anesthetics remain the cornerstone of dental pain control, with 2% lignocaine long regarded as the gold standard due to its safety, reliability, and predictable clinical performance. Articaine, a newer amide local anesthetic with a unique thiophene ring and an additional ester group, offers several clinical advantages. Its enhanced lipid solubility allows superior diffusion through soft tissues and dense cortical bone, making buccal infiltration anesthesia more effective, even in mandibular molars. Unlike lignocaine, articaine undergoes rapid plasma metabolism in addition to hepatic metabolism, resulting in a shorter half-life and reduced risk of systemic toxicity—an important benefit in pediatric patients. Both agents act by blocking voltage-gated sodium channels, preventing nerve impulse conduction.
