Clinical Integration, Accountable Care and Population Health

Clinical Integration: Accountable Care and Population Health, Third Edition, provides a fresh set of insights and new information on the evolution of clinically integrated networks (CINs) across the United States (US) healthcare system. The third edition transitions from the original collaborative work of its four original authors to a 14-chapter anthology that provides a refresh on original issues and introduces a number of new topics including patient engagement, care coordination, and behavioral health. All of which are critical to the future growth of CINs. Chapter 8, Coordinating Care- Transforming the Delivery Process, provides fresh insights to the challenges that are unique to care coordination (and distinct from case management). An evidence-based perspective is provided that draws upon healthcare reform issues and the focus of coordinating care to help every CIN work toward achieving the “Triple Aim”—improving the quality of care for the individual and the community’s population. The authors illustrate how care coordination is a component of medical management and its effect on population health management.

Michael G. Hunt, DO, FACOP, FAAP, MMI, graduated from the University of Osteopathic Medicine and Health Sciences, Des Moines, Iowa and completed his internship and residency in Pediatrics at William Beaumont Army Medical Center in 1994. After residency, he held several leadership positions including Chief of Medicine and Chief of Pediatrics. In 2005 he joined Mercy, Oklahoma City, to grow his pediatric practice. He advocated for the system to implement an EMR. His participation as physician champion and CMIO helped the organization to implement EPIC in 29 hospitals and 200 ambulatory offices representing more than 50 medical specialties. Dr. Hunt completed his Masters in Medical Informatics at Northwestern University in 2012, and is an associate professor of informatics at St. Louis University. He is Board Certified in Pediatrics, and is an Assistant Professor of Pediatrics at St. Vincent's Hospital and Medical Center and the Frank H. Netter MD School of Medicine at Quinnipiac University. In addition to his current role as CMO/CMIO of St. Vincent's Health Partners, Inc, Dr. Hunt continues to see patients at St. Vincent's Family Health Center. Colleen Swedberg MSN, RN, CNL, obtained her undergraduate nursing degree at the University of Calgary in Alberta, Canada, and Master of Science in Nursing at Fairfield University in Fairfield, CT. She is certified as a Clinical Nurse Leader (CNL) with the American Association of Colleges of Nursing (AACN). She worked in surgical trauma in Calgary and surgical oncology at Tulane Medical Center in New Orleans as well as community health with Tulane's hospital based home care agency, primarily in the 9th ward of New Orleans. In 2004, Colleen joined the team of the first Connecticut branch of a national home health care agency, Bayada Home Health Care, progressing to the clinical lead in 2007 of over 30 multidisciplinary field staff providing skilled services to acutely ill patients at home. Currently Colleen serves as Director for Care Coordination and Integration at St. Vincent's Health Partners in Bridgeport, CT.
