A man is dying on a table, and a doctor has ninety seconds to decide what he is — not who, but what: a bleed or a clot, two diagnoses that demand opposite hands. He cuts before he knows he has chosen. The patient lives. And a question opens that takes eleven movements to follow to its end: what was the doctor, in the moment of the cut, when he acted before he understood?

To Exist Is to Differ is a philosophical dialogue between two voices — one that stays close to the body, the bedside, the cost of a single decision, and one that follows where each case leads when it is carried past the room. Together they pursue a single idea to its limit: that to exist is to hold a distinction open against everything one is not, and to hold it at a cost that is paid continuously, until it can no longer be paid.
