Dr Colin McCartney is Staff Anesthesiologist and Associate Professor in the Department of Anaesthesia at Sunnybrook Health Sciences Centre, University of Toronto, Toronto, Canada. Tony Wildsmith is Emeritus Professor of Anaesthesia at the University of Dundee, Dundee, UK.
