pp.xxii, 667 pages, a very good plus hardback in a very good plus dust-jacket [094766002X]. No date but dj has the book priced $16 when published, so circa 1960s. NOT FROM A LIBRARY and indeed completely clean from any former owner markings, although the top corner of the ffep has been snipped - probably done because a name had been there.
