It is a reading failure.The drawing said something specific about which side of the joint to weld, how deep to prepare it, how far each segment should run, or which face had to be finished flat, and the instruction was read past rather than read. The part comes out clean, square, and wrong, usually at the point in the schedule where fixing it costs the most.The reason is that a weld symbol is not a picture. It is a compressed sentence in which every element has a fixed position, and position carries the meaning.
