Dr. Don Middleton

Where medical expertise meets biblical hope in the journey to freedom.

Dr. Don Middleton received board certification in Family Medicine. He has devoted his entire career to serving Arizona families. After 20+ years in family practice and the past decade in addiction medicine, Don has become known for blending medical expertise, compassion, and a whole-person approach to healing.

He studied Biology and Nutrition at Northern Arizona University before earning his medical degree from Western University of Health Sciences. After completing his Family Medicine residency at Flint Osteopathic Hospital, he returned home to Arizona to begin practice and raise his family.

Don is deeply rooted in his Christian faith and active at Christ Church of the Valley, where he regularly serves on medical mission teams to Kenya and Mexico. His passion for faith-informed addiction recovery led him to establish The Dunamis Initiative and author several books that integrate biblical principles with practical tools for lasting transformation.

Throughout his career, Don has held multiple leadership roles in medical education and community health, including Director of Medical Education at Phoenix General Hospital, co-director of Family Medicine at AZCOM, and founder of the medical clinics at Midwestern University’s Glendale campus.

He currently practices addiction medicine at Meadows Behavioral Health—an internationally respected leader in the field—and serves as the Medical Director of Holdfast Recovery, a Christ-centered rehabilitation provider. He is also the Medical Director for Vital4Men in the Phoenix area - a group of men's health centers. Don serves as the national chairman of addiction medicine for the Christian Medical & Dental Associations, a board trustee for the Arizona Osteopathic Medical Association, and a board member for Crossroads Inc., the largest addiction rehabilitation hospital in Arizona.

Don and his wife Tracy have been married for more than 30 years. They have four grown children and three beloved grandchildren.
