跨入21世纪，随着中国国企改革和城市化进程的加快，城市贫困人口日益增加。到2003年底，全国有3000万城市居民生活在绝对贫困线以下，有30%～40%的贫困居民由于疾病使生活水平跌落到贫困线以下。城市贫困人口无收入或收入低，居住环境差，营养不良状况比较普遍，加上心理压力大，患病率往往高于非贫困者。虽然他们对医疗服务的需求高，但大部分没有被医疗保险所覆盖，只好小病不看变成大病，大病小看以致治疗不彻底。20世纪90年代以来，在美国等发达国家开展了一系列社会资本与健康关系的研究，也有少数研究就社会资本与卫生服务利用和筹资的关系进行了探讨。由于独特的社会经济背景和文化传统，中国的社会资本状况与西方国家有较大差异；虽然今天中国卫生领域对社会资本概念的使用日益增多，但是在本研究开展的那个时期（2006-2007），国内开展的此类研究非常少，而且没有就社会资本与健康公平的关系展开具体的分析。本书在国内外已有研究基础上，通过理论研究和实证分析，探讨了社会资本与城市居民健康公平的关系，提出了利用社会资本途径改善中国城市居民健康公平的相关建议。
