Rear flap of the cover has an address form to send in to request an illustrated biographical booklet on Morley. The address form was filled out by Joan Prindeville of 105 Clement Ave in West Roxbury, Massachusetts. Dust jacket is in a protective cover and missing one large piece from the cover and some small pieces along the top edge, along with the front flap. Signed by Author(s).
