L'origine des bruits du coeur était un sujet alors très discuté, et Pigeaux ne manqua pas de prendre rapidement part au débat. Antoine Louis Jules Pigeaux (1807-1877), né à Senlis, avait étudié la médecine à Paris et se consacrait à la cardiologie. Il s'installa à Paris après avoir échoué à l'agrégation. Il est l'auteur de l'un des principaux traités de cardiologie de cette époque, publié en 1839 (Garrison & Morton, 2751), ainsi que d'un traité sur les maladies vasculaires. Pigeaux base sa théorie sur les lois de l'acoustique et sur des expérimentations animales. Selon lui, les bruits s'expliquent par le choc ou le frottement du sang (contre les parois des ventricules, les orifices et les parois des gros vaisseaux, au moment de la systole, pour le 1er bruit ; contre les parois des oreillettes, les orifices auriculo-ventriculaires et la cavité des ventricules au moment de la diastole, pour le 2ème bruit). Il discute et réfute les autres théories proposées (par Laennec, Bertin et Bouillaud, Magendie, Rouanet, etc.), alors que Rouanet avait attribué avec raison le second bruit au claquement des valvules sigmoïdes. Il faudra en fait attendre les expériences de Chauveau et de Marey pour connaître de façon objective et indiscutable l'origine de ces bruits. Cet article novateur de Pigeaux occupe les pages 174-184 de ce volume. Il est le fruit de deux années de travail. Pigeaux eut recours à la méthode numérique et étudia les mains de 200 patients tuberculeux ainsi que celles d'individus non tuberculeux. Il donne pour la première fois une description précise de l'hippocratisme digital et note qu'il s'agit d'un signe prodromique précieux de tuberculose. Jules Pigeaux (1807-1877) est l'auteur d'importants travaux, notamment en cardiologie (cf. Garrison & Morton, 2751).
