The recipient Alfred S. Crawford wrote "Property of/ Alfred S. Crawford" on the front flyleaf (see photo). This is one of 100 copies on Winterbourne paper, for the members of the Harvey Cushing Society, of which Alfred S. Crawford was a member. The binding--of paper-backed boards, with a printed paper spine label (see photo)--is different from that of the 400 copies for sale, which was bound in a green cloth. In addition the title page imprint of the edition for sale is "Charles C. Thomas", whereas the title page of the edition printed for the members of the Harvey Cushing Society says "Printed for the Society". As for Cushing's inscription, he invariably signed "H. C." for those he knew. He wrote out his name "Harvey Cushing" when inscribing for those he did not know. Signed by Author(s).

"James Babb . . . recalls that Cushing's visits always fired him with an indefinable zeal for building up the Library; and during the summers when H.C. was alone Babb could seldom resist dropping in for a chat at 691 Whitney Avenue on his way home" (Fulton, Harvey Cushing, p. 692). James T. Babb graduated from Yale in 1924. "In the thirties he was active in the Yale Library Associates, serving for a time as its secretary. . . . [in] 1938 he joined the [Yale] University Library as assistant librarian, a job he held until he took over as acting librarian in 1943. In the book devoted to Harvey Cushing's Seventieth Birthday Party on April 8, 1939, Plato Schwartz is mentioned on p. 86 as one of the "friends and pupils" who had sent a birthday message to Cushing. In 1938 the neurosurgeon Olof Sjöqvist had published a book entitled Studies on Pain Conduction in the Trigeminal Nerve: A Contribution to the Surgical Treatment of Facial Pain, hence the meaning of the words in Harvey Cushing's inscription. See Garrison-Morton 4908 for Sjöqvist's 1937 paper on "trigeminal tractotomy" for the treatment of pain in trigeminal neuralgia. At the 70th birthday party for Harvey Cushing in April 1939, Olof Sjöqvist was in attendance. "Olof Sjöqvist of Stockholm, who had been spending a year in New Haven as a Rockefeller Fellow, presented a birthday volume of papers written by Cushing's friends and admirers in Sweden. . . . Sjöqvist's remarks in presenting the Scandinavian birthday volume were, in best Swedish tradition, most felicitious" (Fulton, Harvey Cushing, pp. 706-07).
