In this research work questionnaire survey having various reasons of rework was conducted, Survey was carried out in Surat City south Gujarat Region. From the various Literatures 48 Causes of rework were finalized under 7 Major Group which was checked for their impact in construction using IMPORTANCE INDEX (IMPI) method and top most crucial causes were used to develop FIELD REWORK INDEX (FRI). The FRI score between 14 to 30 was approaching stage, 30 to 45 was normal vigilance stage and 45 to 70 was rework alert stage.
