The first half of the book describes the development of the field of minimally invasive cataract surgery, emphasizing the substantial reduction in the induced cylinder and the increase in the percentage of patients with an uncorrected visual acuity of 20/40 or better shortly after phacoemulsification cataract removal with self-sealing wound design. While most contributing surgeons favor 3- to 4-mm incisions, Leroy Bloomberg, MD, describes his 7-mm self-sealing incision. Interestingly, he reports less surgically induced astigmatism than the other contributing surgeons, who uniformly indicate greater amounts of induced cylinder with larger self-sealing incisions.
