Handwritten on blue paper, previously folded, measures 6.5" x 5.25". Solid hinges, text legible. The doctor who performed the procedures was named "D.C. Heigbee." Procedures include the following: Visit boy Wm, prescribed quinine - $2.50; Five visits girl Nelly / pres. Sunday medics - $10.00; services boy Sam: swelled testicle - $10.00; etc. The document notes that payment was received on "Jany. 29th 1857," for a total bill of $22.50. Seller Inventory # 15847
