"Nur die Möglichkeit, die grosse Anzahl der lange Zeit vorher operierten Patienten persönlich untersuchen zu können, erlaubte mir, zu nachstehenden Schlüssen in einigen Fragen zu kommen, wie bezüglich Wirkung der Adhäsionen, Verknöcherung des Defektes, Cystenbildung etc. Die Hauptaufgabe, die ich mir auf Rath von Professor Kocher stellte war die Frage - ob Adhäsionen und Narben, die sich nach einer Craniectomie entwickeln mit schädlichen Folgen für das Gehirn verbunden sind. Die Resultate die mir die Tierexperimente lieferten, verglich ich mit den Angaben von circa 30 Kranken, die - meisten vor vielen Jahren - von Professor Kocher operiert worden waren. In dem ich diese Frage zu lösen suchte, richtete ich selbstverständlich meine Aufmerksamkeit auf den Process der Verknöcherung der Operationsdefecte. Es gelang mir, die grosse Bedeutung der Dura im Process der Regeneration der Schädelknochen durch neue Beiträge zu belegen. Die experimentelle Beobachtung erhielt ihre volle Bestätigung durch die Untersuchung der alten Trepanationsdefecte bei den früheren Kranken der Berner Klinik. Die Nachforschung des Heilungsprocesses von Trepanationsdefecten und der Ursachen der Recidive der epileptischen Anfälle gab mit die Idee eines neues Operationsverfahrens, welches, wenn nicht für immer so doch wenigsten für längere Zeit "Entlastung" des intercraniellen Druckes zur Folge haben könnte. Professor Kocher, nach dem er meiner Arbeit aufmerksam folgte, wandte die neu vorgeschlagene Operation in einigen Fällen genuiner Epilepsie an." Chir. 2: pt.II, pp.9-18) with bases for several techniques. For the local relief of tension Kocher made a small bony opening, split the dura mater, or excised it. For the relief of general pressure due to a cyst or fluid in the ventricles he used subcutaneous drainage or drainage by a silver tube to the exterior. For other cases of general pressure he used a technique patterned after Lannelongue. After turning an osteoplastic flap, he excised the margins of the bone and closed the scalp. In some cases he excised the dura mater beneath the bony defects. Thus he provided a mobile lid at the side of craniotomy. Chir. 26: p.891) supported the principles that both bone and the dura mater beneath the bony defect should be removed, and Kocher (Textbook operative Surgery (1903)) subsequently sutured the dural flaps over the edges of the bone to the overlying soft tissues ."
