This is surprising in view of the fact that their favorable effects in angina pectoris were described by Brunton and by Murrell in the Lancet more than 100 years ago. As expected, a host of new information has been collected since the two previous symposia on nitrates held in Stockholm in 1975 and Berlin in 1978. New insights were gained into the pharmacology, pharmacokinetics and pharmacodynamics of nitrates, as well as into their clinical effects in acute and chronic ischemic heart diseases and in severe congestive heart failure. Relatively little progress, however, was observed in research into the basic action of nitrates. Although most investigators agree that intracellular sequestration of calcium is probably the main mechanism by which nitrates lead to the reduction of vascular smooth muscle tone, the exact site of their action still remains undefined. In contrast, dose-dependent differences in venous and arteriolar tone have long been clearly established. Treatment was again in the main stream of discussion. The question of tolerance following long-term administration was discussed in depth and the term 'pseudotolerance' was introduced to describe the adaptation of the body's circulatory system to chronic vasodilation. This is especially important in long term prophylactic antiischemic treatment in stable, as well as in unstable angina pectoris (i.e. during increased vasomotor tone - spasm).…

It goes without saying that Adalat has changed the company as a whole in its research, development, and marketing, both in Germany and, most importantly, world wide. In research and development, Adalat has led the way in to new areas such as hypertension, acute neurology, dementia, atherosclerosis, and heart failure. In pharmacology, Adalat was present at the dawning of the second phar macological revolution, which started out from whole animal models and progres sed, via in vitro screening and pharmacological research on the DHP receptor, down to the molecular level with the cloning of calcium-channel proteins. Having followed the Adalat story closely in its early years as a bench scientist, let me take this opportunity to thank the inventors, the scientists, and the investigators who have helped to tap the broad potential of calcium-channel blockade with dihyd ropyridines in general and Adalat in particular. But, success should not obscure our vision. There is no reason for us to rest on our laurels: it will take true innovations in cardiovascular research to top the gold standard set by Adalat. Dies ist iiberraschend, wenn man bedenkt, daB bereits vor iiber hundert Jahren Brunton und Murrell im Lancet iiber die giinstige Wirkung dieser Substanzen bei der Angina pectoris berichtet haben. Seit den beiden vorausgegangenen Nitrat-Symposien, in Stockholm 1975 und Berlin 1978, sind - wie erwartet - eine Menge neuer Erkenntnisse gewonnen worden. Dieses Wissen betraf Pharmakologie, Pharmakokinetik und Pharmako dynamik der Nitrate, besonders aber auch ihre klinischen Wirkungen bei akuter und chronischer Ischamie des Herzens und bei schwerer dekompensierter Herz insuffizienz. Daran gemessen war der Fortschritt bei der Erforschung des moleku laren Wirkungsmechanismus der Nitrate eher bescheiden. Obwohl die meisten U ntersucher iibereinstimmen, daB Anderungen von intrazellularem Calcium wahrscheinlich der Hauptwirkungsmechanismus sind, durch welchen Nitrate zu einer Abnahme des Tonus glatter GefaBmuskulatur fiihren. Der genaue art ihrer Wirkung ist noch unbekannt. 1m Gegensatz dazu sind die dosisabhangigen Unterschiede der Wirkungen auf das venose und arteriolare GefaBsystem seit langem gut bekannt. Die Behandlung mit Nitraten stand erneut im Mittelpunkt der Diskussionen. Ausfiihrlich wurde die Frage der Entwicklung einer Toleranz nach Langzeit Therapie mit Nitraten diskutiert. Dabei wurde der Ausdruck 'Pseudo-Toleranz' zur Beschreibung der Anpassung des Kreislaufsystems an die chronische Vasodila tation verwendet. Diese chronische Vasodilatation ist fiir die Langzeit-Prophylaxe sowohl bei stabiler als auch bei instabiler Angina pectoris (d. h. bei Zustanden mit erhohtem Vasomotorentonus-Spasmus) von besonderer Bedeutung.
