Probleme der Diaguose und Therapie des Morbus Hirschsprung Die Hirschsprungsche Erkrankun…g gilt allgemein als ein wohl definiertes und typisches Krankheitsbild. Fiir die Behandlung stehen die Resektionen nach SVENSON, REHBEIN und DUHAMEL zur Verfiigung. Die DiagIiose~ wird klinisch und rontgenologisch gestellt und durch eine Probeexzision aus dem Rektum, die die volle Dicke der Rektalwand erfassen muB, bestatigt. Dies ist derzeit allgemeine Auffassung. Bei naherer Durchsicht der Literatur zeigt sich jedoch, daB nach Tab. 1 (Zusammenstellung von 12 Autoren oder Autorengruppen) in 8-16% der Falle mit atypischen Krankheitsbildern zu rechnen ist, daB die Moglichkeit der Fehl diagnose absolut besteht, wobei vor allen Dingen ein Megacolon rontgenologisch nicht obligat beobachtet werden mull, das typische rontgenologische Erscheinungs bild erst spater, jedenfalls nicht im Neugeborenenalter auf tritt, daB relativ oft das Rektum isoliert dilatiert sern kann, was wir auch beobachten konnten und daB es vor allen Dingen histologisch anscheinend verschiedene Typen gibt. Die Extremfalle des ultrakurzen und des langen aganglionaren Segmentes. weisen ebenfalls besondere Probleme auf. Die verbesserten Moglichkeiten der histologischen Diagnosestellung und die Notwendigkeit sich mit dem Morbus Hirschsprung des Neugeborenenalters zu befassen, schienen ebenfalls geniigend Begriindung die diagnostischen Probleme neu zu diskutieren. Zusammenfassung der Ergebnisse Die Diagnose Morbus Hirschsprung kann grundsatzlich rontgenologisch, funktionell (Druckmessung) und histologisch gestellt werden.
