Ce texte novateur a été détaché d'un volume des Mémoires de l'Académie de Médecine ; il n'a pas fait l'objet d'un tirage à part. *** A LANDMARK EVENT I…N THE HISTORY OF ANORECTAL MALFORMATIONS *** Jean Noël Roux (1797-1869), souvent surnommé Roux de Brignoles, fut chef de clinique de Delpech à Montpellier, exerça ensuite la chirurgie dans le Var (à Saint-Maximin puis à Brignoles), puis devint chirurgien des hôpitaux de Marseille et professeur de médecine opératoire à l'Ecole de médecine de Marseille, membre de l'Académie de Médecine. Il rapporte ici l'observation d'un nouveau-né dont " l'anus et le méat urinaire était imperforé", " l'anus étant fermé par la peau, dure et épaisse". Jusque-là, ce type de malformations était traité par une simple ponction avec un bistouri, une lancette ou un trois-quarts. Roux démontre la nécessité de réaliser "une dissection lente et méthodique des muscles du périnée et de l'anus, dans leur ligne médiane" donnant accès à "une couche de tissu cellulaire plus ou moins épaisse, derrière laquelle le doigt indicateur peut rencontrer l'intestin qu'il faut ouvrir." Roux exprime ensuite son opinion "sur la conformation de l'extrémité du rectum" chez son petit malade, et "l'explication du passage momentané d'une partie des matières excrémentielles par le canal de l'urètre." Roux indique que le défaut de développement doit habituellement être attribué à une déviation ou une diminution de calibre de la branche de l'artère honteuse, et il insiste sur l'importance des muscles elliptiques du sphincter et releveurs de l'anus, "pourvu que par une dissection bien faite, on leur rende la liberté d'action qui dépend de la désunion de leur bord inférieur, à travers lequel l'anus artificiel doit être établi." La planche montre la configuration anatomique de la malformation et le trajet du bistouri. By 1860, Bodenhamer championed the midsagittal incision first described by Roux 27 years earlier. The 1980s ushered in another procedure, the posterior sagittal anorectoplasty (PSARP), resurrecting the concept initially proposed by Roux in 1834 and subsequently used by Bodenhamer in 1860.
