In der folgenden im Jahre 1885 begonnenen Studie mache ich den Versuch, gestützt auf ein auf eigener Beobachtung beruhendes grosses Material chronischer Fälle, die nervösen Symptome beim chronischen Gelenkrheumatismus zusammenzustellen und zu erklären. Bis jetzt ist ein derartiger Versuch meines Wissens noch nicht angestellt worden. In der Litteratur finden sich nur vereinzelt Andeutungen der Autoren oder Aussprüche über einenZusammenhang des chronischen Gelenkrheumatismus mit dem Nervensystem vor; ausführliches dagegen fehlt vollkommen. Auch ich will nicht behaupten, das Thema erschöpft zu haben. ? Ich knüpfe an ein paar ältere Aussprüche bekannter Autoren an und versuche die geschilderten Symptome auf das Rückenmark zu beziehen ? ob und mit welchem Recht, lasse ich dahin gestellt. Deshalb gehe ich aber auch nicht so weit zu behaupten, der chronischeGelenkrheumatismus sei eine Rückenmarkskrankheit. Ich bescheide mich vielmehr damit, die nach meiner Ansicht vorhandene Mögli chkeit eines causalen Zusammenhangs mit diesem Organ zu zeigen. Trotzdem wird sich wohl von mancher Seite ein Widerspruch hiergegen erheben. Es lassen sich ja manche Dinge, die ich vorbringe, auch anders erklären, und manches nicht von mir Berührte deutet ebenfalls auf andere als nervöse Einflüsse hin. Ich erinnere nur an die auch von C. Gerhard beschriebenen rheumatoiden Gelenkerkrankungen der Bronchiectatiker. ? Dagegen würden die bei Rheumatikern zum Teil angenommen, zum Teil als vorhanden erwiesenen abnormenZusammensetzungen des Blutes und besonders des Urins sich am ehesten auch durch einen nervösen Einfluss erklären lassen. Ich verweise auf das, was Professor Rudolf Arndt in seinem Werke ?die Neurasthenie" über die Dysekkrisieen des Urogenital-Apparates und speziell über die Paruresie (pg. 88) sagt: Man muss zu ihnen auch die reichliche Ausscheidung von Harnsäure rechnen, die gerade beinervösen Menschen zeitweilig eine sehr bedeutende zu sein pflegt, und bekanntlich Symptom einer Constitutionsanomalie ist, die als gichtische wahrscheinlich eine viel grössere Rolle spielt, als man vornehmlich in der neueren Zeit anzunehmen geneigt war." Und an anderer Stelle (pg. 157) desselben Werkes spricht sich R. Arndt dahin aus: "Der acute Gelenkrheumatismus - was für eine Krankheit er zuletzt auch immer sei, ob eine Infektions-, eine Refigerations- oder Constitutionskrankheit, betrifft wohl nur Neurastheniker; wobei aber wieder darauf hingewiesen werden muss, dass selbige sehr wohl gut genährt, selbst üppig erscheinen können, ohne im Geringsten stark und kräftig zu sein. Der chronische, insbesondere der Muskelrheumatismus befällt vorzugsweise schwache und widerstandslose Subjekte, die zu reichlicher Harnsäurebildung hinneigen." Nach R. Arndt's Ansicht ist der Rheumatismus nur als einAusgang der Neurasthenie anzusehen, d. h. er entwickelt sich auf Grund eines anomal gebauten und somit anomal funktionierenden Nervensystems. Das Rückenmark aber, mit dem ich den chronischen Gelenkrheumatismus in Verbindung zu bringen suche, ist nur ein Teil des Nervensystems. ? Ich führe diese Dinge hier an, um zu zeigen, dass eine Berechtigung, das Thema, den chronischen Gelenkrheumatismus in seinen Beziehungen zum Nervensystem, zum Gegenstand einer Studie zu machen, mir nicht abgesprochen werden kann."
