The author places today's disparities in a comprehensive historical context, which highlights the simultaneous progress and intractability of the issue. The author demonstrates how contemporary inequality and injustice in healthcare provision in the United States arose from deliberate decisions by powerful actors. From the exclusion of women and people of color from medical education in the 19th century, through systemic racism, to contemporary resource allocation, this work demonstrates the lasting impact of these decisions on healthcare provision and outcomes. The author concludes with an examination of reform and resistance from both above and below in an insightful analysis of how the social, economic, and political forces that have shaped healthcare inequality continue to play out in the present day.
