Add to basketCondition: Sehr gut. 251 S., Guter Zustand. -- Das vorliegende kleine Buch hat die Aufgabe, auf häufige psychiatrische Fehldiagnosen in der allgemeinärztlichen Praxis aufmerksam zu machen und will versuchen, solche Fehldiagnosen vermeiden zu helfen, indem es eine Einführung in die psychiatrische Differentialdiagnostik vermittelt…. Verschiedene Wege der Darstellung des Stoffes boten sich dazu an. Man konnte beispielsweise daran denken, die wichtigsten vorkommenden psychiatrischen Krankheiten nach dem Schema der speziellen klinischen Psychiatrie nacheinander lehrbuchmäßig aufzuführen, ihre Hauptsymptome zu schildern und dann ausführlich auf die bei den einzelnen Krankheiten besonders oft beobachteten Fehldiagnosen einzugehen. Ich glaube, daß eine ganz kurze gesonderte Darstellung der Systematik der heutigen Psychiatrie, die natürlich auch im laufenden Text immer wieder diskutiert werden muß, in der Tat auch in diesem Rahmen unentbehrlich ist, aber ich möchte sie an den Schluß dieser Fibel stellen. -- Eine weitere Gliederungsmöglichkeit könnte nicht von psychiatrischen Krankheitsbildem, sondern von der allgemeinen Psychopathologie ausgehen. Man könnte also Störungen beispielsweise des Wahrnehmens, des Denkens, des Fühlens, Strebens und Wollens, des Icherlebens, des Gedächtnisses, der Reaktionsfähigkeit, der Intelligenz und schließlich der Persönlichkeit schildern und dann aufführen, bei welchen Krankheiten derartige Störungen Vorkommen und wo und wie ihre Feststellung und Bewertung Anlaß zu diagnostischen Irrtümern geben kann. Hier hätte man zweifellos eine bewährte psychologische Ordnung zur Hand, aufbauend auf der in der klinischen Psychopathologie gebräuchlichen Gliederung des Stromes des normalen und abnormen seelischen Erlebens in die "Arten", die "Grundeigenschaften" und den "Hintergrund". Wir werden im Lauf der Darstellung in der Tat sehen, daß zahlreiche hier erörterte Krankheitsbilder sehr entscheidend durch ganz bestimmte psychopathologische Störungen einzelner dieser summarisch aufgeführten Kategorien geprägt sind. Warum wir uns zu einer solchen systematischen Aufteilung des Stoffes nicht entschließen konnten, hat seinen Grund darin, daß innerhalb der einzelnen für die Diagnosenstellung und die diagnostischen Irrtümer wichtigen Symptombilder sehr oft auch Störungen aus verschiedenen anderen Bereichen vorliegen, so daß wir doch stets mehr oder weniger "das Ganze" zu berücksichtigen hätten, um dem wirklichen Leben gerecht zu werden. Außerdem blieben manche Kategorien dieser psychologi-s chen Ordnung für unsere auf die Praxis zugeschnittenen Fragestellungen leer. Für den größten Teil des Textbuches habe ich deshalb einen anderen, unsystematischen Weg gewählt, und zwar habe ich versucht, rein der Häufigkeit nach besonders vordringlich in der Sprechstunde geklagte Beschwerdebilder und Symptomverbände zusammenzustellen. und die differentialdiagnostischen Erwägungen zu entwickeln, die der Arzt, ohne den Apparat einer Fachklinik zur Verfügung zu haben, in Betracht ziehen muß. Daraus ergibt sich dann im einzelnen von selbst, welche Irrtümer besonders naheliegen und welche Möglichkeiten bestehen, ihnen zu entgehen. Der Nachteil dieser Art von Darstellung ist vielleicht eine gewisse Willkür in der Auswahl und Gliederung. Ich kann nur hoffen, daß dieser Mangel durch die Praxisnähe insofern aufgewogen wird, als ich vom einzelnen Kranken und seinen vorgebrachten Beschwerden und den in der Sprechstunde erhebbaren Befunden ausgehe, so wie wir dies täglich im Sprechzimmer oder in der Wohnung der Patienten erleben. Auf den systematischen Ort der einzelnen diskutierten Symptome und Syndrome wird dann in der kurzen psychiatrischen Nosologie des anhängenden zweiten Teiles noch einmal eingegangen. -- Wenn ich mich also bemühe, von der Häufigkeit der in Frage kommenden Sachverhalte auszugehen und zunächst diejenigen Symptome und Symptomgruppierungen (Syndrome) behandele, die dabei besonderen Anlaß zu diagnostischer Verkennung geben, so kann doch dieses Prinzip ni.
