Es gibt wohl kaum eine Operationsmethode an der Gallenblase und am Gallengangssystem, die nicht zumindest auf theoretische Konzeptionen Langenbuchs zurückgeht. Bereits 1884 schreibt er in der Berliner klin. Wochenschrift: 'Handelt es sich um einen Stein (im Choledochus), so könnte dieser, ohne die umgebenden Weichteile empfindlich zu verletzten, mit den Fingern oder gepolsterten Zangen zerdrückt werden. Gelänge dies nicht, dann wäre die Eröffnung des Duodenums behufs Spaltung der Wand des Diverticulum Vateri in Aussicht zu nehmen. Dieses Verfahren wäre auch bei der narbigen Stenose und der Anwesenheit von Spulwürmern in der Portio intestinalis angezeigt. Andererseits könnte mann auch den meist kolossal erweiterten Choledochus inzidieren, die ausströmenden Galle mit großen Schwämmen auffangen, dann den Stein extrahieren und die Öffnung wieder durch Naht schließen'. Langenbuch fährt fort mit der Empfehlung, nach der Choledochusnaht zur Entlastung eine Fistel an der Gallenblase oder, falls dies nicht möglich ist, am vorderen Leberrand anzulegen. Weiter heißt es in dieser Arbeit: 'Auch hier (wenn der Choledochusverschluß nicht beseitigt werden kann) würde eine Verbindung des Darms direkt mit der Leber oder dem Choledochus zu planen sein'. Damit waren Anregungen gegeben für die Choledocholithotripsie, die Choleodochotomie, die Duodenocholedochotomie (Sphinkterotomie), die Choledochoenterostomie, die Hepatostomie und die Hepatoenterostomie. - G.Skibbe.
