unbound. Document signed -- a permission form filled out by hand in red ink by the American photographer, requested by Walker Evans to illustrate an essay on photography. Accompanied by a one-page typed note signed, stapled to the form. The form measures 11 x 8.5 inches; the note measures 7 x 6 inches and is written on personal letterhead, Mill Valley, California, November 22, 1968, in part: "I enclose the print you asked for in connection with Walker Evans' essay." Faint paperclip stain near the top left corner of the note. Very good condition.
