Faint owner signature on the front cover (we can make out "Cincinnati, Ohio") and a tiny penciled doodles on the lower wrap. Wrappers tanned with some wear and soil, two very light vertical creases more or less throughout, and a small shallow abrasion on the lower wrap affecting a few words of text, overall a nicely preserved, very good copy. The first 15 pages are dedicated to James Murphy's "Thesis on Insanity"; he was studying for his M.D. at the University of the Pacific, the first medical school on the West Coast. In the first paragraph, he makes the interesting claim that "[i]t is shown, by the statistical reports of the various insane asylums throughout the Union, that California furnishes more cases of Insanity, in proportion to her population, than does any other State.," and that the state's percentage of recoveries is just as bad. He discusses the causes of "insanity" and, regarding to California, dedicates a paragraph to the competing arguments of an under- or overcharging of electricity being a cause; he believes that it is more likely an excess of oxygen, as well as "blood-poisoning" from the consumption of alcohol. Then: "Among the exciting cases, *Onanism* stands most prominent in California, from which cause we have the largest number of cases in our asylum [Stockton Asylum]." Murphy also faults gambling, "libertinism," celibacy, an excess of devotional feeling, and the profession of miner: not because of the deplorable working conditions, but "[t]he exciting and depressing effects produced by the sudden loss or gain of wealth, while the stooping position in which he is obliged to work, causes congestion of the brain, and, consequently, inflammatory diseases." Thankfully, "Literary Pursuits are said to be a cause, but statistics do not maintain the saying." Murphy goes on to define the five classifications of "insanity" and gives descriptions of symptoms, followed by a brief text on diagnosis and about two pages dedicated to treatment. Some of the more colorful treatments include: cups or leeches to the back of the neck, drugs to induce sweating or vomiting, and "Counter-irritation, by means of blistering" to a shaved head or along the spine: "and it should be pretty extensive, as small blisters seldom accomplish anything." While we couldn't help but discuss the most wildly outdated methods for working with patients, Murphy's understanding of mental illness is primitive but not without compassion. He describes the virtues of cleanliness and the removal of stressors, and he is unequivocally opposed to the use of mechanical restraint. In the final paragraph he writes that "[i]n all intercourses with the insane, we should be kind but firm ." Other topics covered are follicular diseases of the throat, in a piece by W.W. Ward of San Francisco; "Radical Cure of a Case of Fistula in Ano, Without the Use of a Knife," by D. B. Hoffman of San Diego; a sanitary report of Fort Churchill, Nevada Territory, by Chas. C. Furley; and "Abscess of the Frontal Sinus" by L. C. Lane of San Francisco. The magazine also prints a couple of translations from foreign publications and a review section, which reports on the inaugural issue of the *American Journal of Ophthalmology*. The interior of the wrappers print full-page advertisements for two San Francisco druggists: Geo. S. Dickey (where leeches may be purchased, among much else) and H. P. Wakelee. A fascinating glimpse into the Civil War-era study of mental illness and medicine in California. Issues are very uncommon. It appears that *The San Francisco Medical Press* published about 20 issues (costing a hefty 75 cents apiece) before merging with the *Pacific Medical and Surgical Journal* in 1865.
