Mit dem Begriff mitochondriale Myopathien bezeichnet man eine Gruppe von seltenen Energiestoffwechselerkrankungen, die im engeren Sinne auf Defekte in der mitochondrialen ATP-Synthese zurückzuführen sind. D…ie Krankheitsbilder weisen eine groÃe Palette von Symptomen auf: neben einer proximalen Muskelschwäche beobachtet man sehr häufig eine Ataxie, eine Myoklonusepilepsie, Erregungsüberleitungsstà rungen des Herzens, cerebrale Ausfälle, Hà rstà rung, Augenmuskelschwäche, Retinitis pigmentosa, Diabetes mellitus sowie Kleinwuchs. Dabei kann man bei den Patienten oft ein erhà htes Liquoreiweià feststellen. Neben spontan auftretenden Erkrankungen werden überwiegend familiäre Fälle beschrieben, die einen ungewà hnlichen Erbgang aufweisen: unabhängig von den Mendelschen Regeln wird die Erkrankung nur von der Mutter auf ihr Kind übertragen ('maternale Transmission"). Bereits im Jahre 1962 gelang der Nachweis von morphologischen und biochemischen Veränderungen von Muskel-Mitochondrien in Patienten mit mitochondrialen Myopathien (Luft et al., 1962). Histologisch sind die Krankheitsbilder meist durch sogenannte 'Ragged-Red Fibers" charakterisiert, die Muskelfasern mit abnormen subsarkolemmal gelegenen Mitochondrienanhäufungen darstellen. Im letzten Jahrzehnt wurden erste Erfolge bei der Aufklärung der mitochondrialen Fehlfunktionen erzielt. Dies gelang vor allem durch eine intensive Charakterisierung der Enzymdefekte in Gewebehomogenaten dieser Patientengruppen (z.B. Reichmann und Wildenauer, 1991; Reichmann et al., 1992; Reichmann et al., 1986). Von den biochemischen Veränderungen in den Mitochondrien sind neben dem Lipid- und Pyruvatstoffwechsel in erster Linie die Enzymkomplexe der Atmungskette, und damit der ordnungsgemäÃe Ablauf der oxidativen Phosphorylierung betroffen. Seit 1988 weià man, daà Stà rungen der oxidativen Phosphorylierung durch mitochondriale DNA-Variationen hervorgerufen werden kà nnen (Holt et al., 1988), und als Ursache neurologischer, internistischer, paediatrischer und ophthalmologischer Krankheitsbilder angesehen werden müssen.
