Tollerata in numerosi luoghi e differenti circostanze per gran parte dell'antichità e del Medioevo, l'omosessualità ha dovuto affrontare, nel corso dei secoli successivi, una lunga ondata di intolleranza che giunge ai nostri giorni. Divenuta una tara, se non addirittura una perversione, doveva essere combattuta come una malattia. Una malattia da eradicare con qualunque mezzo. Nel corso dei decenni le tecniche sono state talvolta relativamente blande (ipnosi, psicoanalisi), altre volte violente (lobotomia, terapia ormonale, scosse elettriche). Centinaia di migliaia di persone sono passate attraverso questo orribile calvario. Non una di loro ha effettivamente modificato il proprio orientamento sessuale. La teoria che sta alla base di queste supposte «cure» è quella secondo cui l'omosessualità sarebbe un comportamento «deviante», «patologico», «aberrante» e, soprattutto, appreso. D'altra parte il Diagnostic and Statistical Manual of Mental Disorders - il manuale internazionale di riferimento delle patologie mentali - ha cancellato l'omosessualità dalle patologie sessuali solo nella sua quarta edizione, nel 1987. L'analisi scientifica della tematica omosessuale è recente e relativamente poco conosciuta. Ancora oggi gran parte delle persone ritiene che l'orientamento eterosessuale o omosessuale sia il risultato di un apprendimento, dovuto alle interazioni sociali che si sono avute nella prima infanzia. La scuola freudiana e postfreudiana ha profondamente influenzato la società, additando nel rapporto coi genitori la causa di uno sviluppo psichico «sbagliato». Ma studi accurati, ormai piuttosto solidi, di biologia, genetica e neuroendocrinologia puntano con sempre maggior convinzione a una spiegazione biologica dell'orientamento sessuale. Più le conoscenze avanzano, più appare chiaro che omosessuali (o eterosessuali) si nasce, non si diventa. Ma per gli omosessuali, data la diffidenza ancora forte nella società e l'omofobia dilagante, questo significa spesso dover riconoscere la propria natura al prezzo di grandi sofferenze, sensi di colpa e recriminazioni. Una migliore comprensione dei meccanismi biologici che stanno alla base dell'orientamento sessuale può dunque portare a un'accettazione più ampia dell'omosessualità nella società e ridurre così le sofferenze inutili che troppe persone hanno patito per troppo tempo. Jacques Balthazart - studioso di neuroendocrinologia legata alla sessualità - ha scritto "Biologia dell'omosessualità" per questo: rendere accessibili a chiunque i dati più aggiornati della letteratura scientifica sull'orientamento sessuale, e per correggere le concezioni sbagliate, e queste sì aberranti, ancora così tanto diffuse nella nostra società.
